Viewing Policy membership
To view policy membership
On the My Self menu, under My Details, click Policy membership.
The Policy membership page appears.
In the Insurance type drop-down box, select the type of the insurance you want to view.
In the Policy no drop-down box, select the policy number categorized under the insurance type that you want to view.
The following details appear:

Displays the sum of amount insured for |
TPA |
Displays the information whether Third Party Administrator (TPA) handles any claims |

Displays the name of the TPA |
Information of the members insured under the policy |

Displays the name of your family member insured under the policy |
Relationship |
Displays your relationship with the family member |
Gender |
Displays whether male or female |
Date of birth |
Displays the date of birth of the family member |
Blood group |
Displays the blood group of the family member |
Membership id |
Displays the membership id of the family member |
Premium amount |
Displays the premium amount being paid towards the insurance policy |
Covered |
Displays whether the member is covered under the insurance policy or not |
Nominee |
Displays the information whether the member is a nominee for the insurance policy |
Contribution (%) |
Displays the percentage of the contribution allocated to the nominee |
Note 1: Fields marked * are mandatory, and they cannot be left blank.
Note 2: Fields that appears dimmed cannot be modified.
Note 3: All the details under My Details are provided by you at the time of joining, which are uploaded by the
respective HR department. You should contact the respective HR representative for any
updates or corrections in details
