Exit Feedback

As an administrator, the exit feedback forms for various designations can be created, edited or deleted.

    1. On the PW Admin menu, under Separation, click Separation .

    2. On the left navigation pane, click Exit Feedback Form.

    3. On the Exit Feedback page, Name, Description, Active, Add/edit question and preview fields can be seen

 

To Add feedback:

      1. Click button

The Add Exit Feedback Form dialog box appears.

      1. Fill up the respective details in the given  fields like Name, Copy Template and Description textbox

      2. Perform one of the following steps

        1. To save the exit feedback form and close the dialog box, click Save and Close

        2. To save the form and continue adding details for another exit feedback form, click Save and Continue

      3. Once you save and close feedback form, your Exit Feedback entries are reflected in the list of Exit feedback Form page

        1. To save the form  and continue adding details for another exit feedback form, click Save and Continue

 

Field

Description

Name

Displays the name of the employee

Description

Displays description of feedback details provided by the employee.

Active

Displays whether the  reason for Separation is Active  - Yes or No

If Yes, the reason is available for selection in the applicable submenus, else it

is not available

Add/Edit Question

Displays the Exit Feedback Questionnaire, also questions can be added or edited by the employees

Preview

Displays the preview Feedback Questionnaire with answers
